Summer Day Camp Forms
To apply for enrollment at Bluebonnet School’s 2006 Summer Day Camp for school-aged children, download the Bluebonnet 2006 Summer Day Camp Application, print it out, complete it and mail or bring it to us with the appropriate fees.

When your child is accepted for enrollment in summer camp, you will need to fill out and submit the Bluebonnet Registration and Re-Enrollment Form and the Bluebonnet School-Age Health Form before your child may attend.

Infant through Kindergarten Forms
To apply for enrollment in one of Bluebonnet’s programs from infant through kindergarten, download the Bluebonnet Preschool/Afterschool Application Form, print it out, complete it and mail it or bring it to us with the $25 nonrefundable application fee.

To apply for enrollment in Bluebonnet’s afterschool program download the Bluebonnet Preschool/Afterschool Application Form, print it out, complete it and mail it or bring it to us with the $25 non-refundable application fee.
